The action of lasers on acne will be done either by a reduction of the bacterial load or by the reduction of the size of the sebaceous glands and the production of sebum.
For the antibacterial action we find blue, red, pulsed lights and vascular lasers. It can be admitted that there is little evidence of their effectiveness based on the criteria of “evidence-based medicine” and that these techniques are little used.
For those looking to destroy the sebaceous glands, we can remember infra-red lasers and dynamic phototherapy (PDT).
Among the lasers we find Smoothbeam (1450 nm) and Aramis (1540 nm) which cause fibrosis of the sebaceous glands comparable to that which we have with isotretinoin but rather superficial with these lasers. There are few serious publications on these techniques, but there are good clinical results, especially in adult women with resistant micro-cystic mandibular acne.

With PDT a photosensitizing product is applied for 3 hours on the skin and then the area is irradiated with a red light.
Under the effect of illumination, the cream becomes active. This technique seems quite effective, especially on inflammatory lesions, but it causes quite significant side effects and remains painful. We will wait for the proof of this effectiveness and today the lasers used in acne are often content to improve the residual scars which can be considered as a failure since the goal is to avoid their appearance.
